Extern Outreach Support Service (EOSS) is in operation since May 2018. At Extern we believe supports should evolve as an individual’s needs change and they move forward with their lives. We aim to support individuals to be independent and become active members of their community. In partnership with the HSE Disability Services we encourage people to have a voice and a say in their service so that their immediate and long terms needs can be addressed. The EOSS in partnership with the HSE Disability Services work with people with a disability and/or autism or who may find it challenging to integrate into traditional social, educational or community systems. EOSS is aligned with the HSE New Directions Principles, such as person centeredness, community inclusion and active citizenship. The programme supports individuals to identify areas of potential growth that they would like to explore; a key worker will help them identify steps to achieve these goals in a collaborative way.
